How to make this slide display in an email

I need your help! I would like the following page (see attachment) to display in an e-mail to show each statement with it's selected check-box. (The check-boxes were inserted from the Insert/Control button.) This way the user can type their email address, and have the selected items display in the email to reference for later. I am not a Java Script expert so not sure what "java script" to type in the trigger dialog box for the "Email" button.
Any guidance would be appreciative.

This should help you. I assigned a value to each checkbox. Seldom gives you a score of 1 and Always gives you a score of 7.

I want the user to click one checkbox per box, and then be able to email the entire slide to themselves. What did I do wrong?

No attachment this time. If you are replying by phone, or email, the system will not accept your attachments. You have to come to this site ona web browser and use the ADD ATTACHMENT button for it to accept your .story file.

Hi Russell, it's me again. When I imported your slide into my deck - I ran into an issue with the 3rd and 4th checkbox not showing up in the email message when one of them was selected. It did not display. The other choices did display in the email.
It showed up like this in the email

You live for facts, and tend to be consistent and logical.
You prefer structure and firm decisions. You like to make lists and plan your day.
When I had tried to import your initial slide in my elearning module - a dialog box displayed saying a variable was the same as what was already in my elearning module so I think that was what caused the issue. When I went back to your initial slide, I renamed the variables (sentence1, sentence2, sentence3, sentence4) and changed them to another name, and reimported it back in to my elearning module then nothing displayed in the email. So, I deleted the slide from my elearning module and went back to your initial slide and removed the new variables I had created and reimported your initial slide again to my elearning module, and the dialog box didn't display telling me to change the variable and seemed to be working til I tested it and then only 3 out of 4 points in the email showed up.
I'm sorry but I can't look at it at this time. I am at home and won't be back to the office until around 8 am Monday morning.

It works! I'm not sure what I did wrong - but thank you!!!! Do you know if there are any classes that teaches more about using java in storyline and creating pages so folks can print or email their results?

I don't know if they have specific classes on this, but you may want to check out what's available on a site such as Lynda.com - they have a lot of training specific to Storyline!
