There isn't a way within Storyline to indicate that the user is also using a screen reader, so you may want to provide instructions to the user to turn down the Storyline volume using the volume button on the player if they'll also be using a screen reader.
You can skip them from the player navigation tab order as detailed here, but not change the order.
As for the alt text, have you assigned it on the master slide? You will not be able to access them from the custom tab order for the purpose of adding alt text or changing their order. The way in which you can remove a master slide object from the tab order or set its alt text is by using the 'size and position' dialog for the shape from within the master view.

1. In storyline we can Customize the Tab Order of Slide Objects, but how to manage the order of focus to player controls? Is it possible?
2. we can not give alt text to elements used in the slide master in the Tab order.
