Hey Rajan, I've been playing around with the file and I believe I've managed to replicate the issue you're talking about once.
I could only see 4 options on the right-hand side as two had overlapped at the top - the issue seemed to raise when I refreshed the slide. This leads me to believe that I think it is a bit of a bug in Storyline. You have a trigger on the try again button which restarts the base layer instead of just hiding the try again layer.
Is this the behaviour you need? Do you want all of the answers to reset on a retry? Or could a learner go back to the question with their previously submitted answers still being there and they just have to reshuffle them?
Just to confirm, if I remove this trigger the bug didn't seem to appear any more:
Important to note that all of the previous queston/answer positions remain as they were and a learner has to reshuffle them. Do you need the question/answers to reset?
Apologies, I went radio silent on this for a little. I'm not sure if you've come up with a solution for this yet. It's not ideal, but adding a loading loop slide seemed to fix the bug - I've run it a few times with this new file and it hasn't appeared again.

My requirement is to reset to original position..i'm using buttons in my course. so that are vital for me.

Glad to hear it works! No trouble as well, you can remove slides from within the player menu.
