Hello. I am new to Storyline but hoping someone can help me out.
I need to be able to input chapter terms into each input box and in no particular order. Then when clicking check answers, an 'x' or check mark will appear. Triggers work fine when checking correct answers but begin marking all incorrect when the conditions are flipped.

I'm not clear, even after looking at the image, what you are creating, and what issue you are experiencing.
With your permission, I'd like to take a look at your project file to investigate what's happening. You can share it publicly here, or send it to me privately by uploading it here. I'll delete it when I'm done troubleshooting.
