I want to include three or four short answer questions on a single slide; the survey question short answer template appears to only have fields for one question and answer, and I am not sure how to modify it to insert additional question and answer fields.

Tell me more about what you had in mind for this slide. Do you want to evaluate the learners' responses as Correct or Incorrect?
As a first step, check out Wendy's tutorial on Evaluating Multiple Fill-in-the-Blanks on One Slide. You could apply the same idea using short answers. That should get you going in the right direction!
