I have faced some issue with Tracking results in LMS.
I published the course in Scorm 2004, 4th edition. However I am getting the tracked results and I can able to see them. But the issue is for one question in which the learner response is missing.
Attached image is reference, in which the learner response is missing for question 21.
Any ideas about this will help me a lot, Thanks in advance.
It looks like that is a multiple choice question? If so, I see it's indicated the user answered correctly - and therefore there is only one option that they could have chosen.
I got the solution for this, If the options have any special characters, the learner response is not tracking in the LMS results. In the 21st question, 2 options have the special charterers and those are not tracking in Moodle as learner options. I am not sure this issue is from Moodle or storyline, if this is about storyline I hope it will be sort out in next version.
scorm 2004 support in Moodle is not great and it is recommended to use scorm 1.2. unfortunately that would mean the quiz question text would not populate the interaction report
Thanks for the update and I can confirm with that piece of information it is something we reported to our QA team in regards to answers that include special characters end up being reported as "undefined" I'll link this thread to the report with our team so that once we have any additional information to share we can post it here.
we do have a couple of bugs open in the Moodle tracker related to special characters not being translated correctly but not for SCORM 2004 packages. If you could turn the SCORM debugger on: https://docs.moodle.org/en/SCORM_FAQ#Debugging
Then capture the errors that occur while the learner uses the package,
then create a bug in the Moodle tracker (tracker.moodle.org) and attach the debugging log and a SCORM package that allows us to reproduce this I might be able to take a look.
It's possible that we might close it as "won't fix" as we aren't doing much further development on SCORM 2004 and mainly only support SCORM 1.2 and AICC packages but if it's an easy fix we might be able to squeeze it in.
No guarantee on time-frames though, SCORM is maintained by volunteers, alternatively you could contract your local Moodle Partner to take a look to see if they can resolve it for you.
Can you please advise how I get access to the data in moodle that shows the user responses for each question?
Also, I noted a comment that scorm 1.2 is preferable over scorm 2004, but that the question text does not populate the interaction report with scorm1.2. Is there another indicator such as question number or do you just need to interpret what question has been asked by looking at the answer text?
This thread is a bit older so I'm not certain that folks are still subscribed here, but you could look at reaching out to them directly by using the "contact me" button on their profile. You may also want to reach out to Moodle to see what options you have for viewing and gathering the data in their environment.
As for which one is better, I think it depends on the version of Moodle you're using, and so I'd defer to your LMS team for advice there.

Hi Krishna
go to the Result Slide properties and make sure there is a tick against Q21 to include as selected question

You could test to see if it Moodle or Storyline by testing in scormcloud

Hi Dan Marsden,
Thanks for support.
At present we have removed the single quoted from the options and now its working fine. If we got any further issues we will keep in touch with you.
Thanks Dan for popping in here, and I'll share that with our team as well so that we're aware it's happening in Moodle as well.
