I'm trying to create a slide that has five Likert scale questions AND an essay survey question - all on the same slide. Most other discussion topics related to "multiple questions on the same slide" are in regards to graded quiz questions, and I'm not sure how/if they apply to this issue I'm facing.
Here's an example of what I'm trying to do... (but it needs to be functional, of course!)
We only have the set up for one question type per slide, but the Likert scale will include more than one row/column per set up - and since essay questions aren't graded - have you looked at just adding a text entry? If you need to capture the learners response and report it to your LMS you may want to use the method here.
Thanks for the response, Ashley - much appreciated! I've set up something based on the article you suggested. I think it will work, and will only update this thread if it doesn't.
I know this thread is pretty old but...thought it'd be worthwhile to ask. How in fact did/do you complete step 1 of adding a short answer survey question to a slide which has a likert question on it already.

Hello Valentina - I'm not sure how Steve was able to set up the two question types on one slide as his original question indicated. I do know that he is still active in the forums, but not sure if he's still subscribed here. You are welcome to reach out directly via the 'Contact Me' option on the user profile if needed. Otherwise, hopefully someone in the community will be able to assist with your design request.
Hi Valentina—I'll be happy to impart my knowledge with you, as well as anyone else who stumbles on this thread.
It's been a while since I set this up and have only done it a couple of more times since. But I've done my best to summarize all the steps below.
Insert your Likert survey slide.
On the Likert survey slide, insert Text Data Entry fields for the short answer questions. (Example: "What was your favorite part of this training?")
For the text entry fields, change the variable names to match the questions being asked, and put the number 1 at the end. (Example: "Favorite_Part_1")
Insert a new essay survey slide. From the Insert Slides window, this is Quizzing > Survey > Essay.
On the essay survey slide, change the essay's variable name to match the question being asked, and put the number 2 at the end. (Example: "Favorite_Part_2")
On the essay survey slide, add a slide trigger to have the essay variable equal the text entry variable from the Likert survey slide when the timeline starts.
On that same slide, add another slide trigger to have the essay be submitted when the timeline starts.
Ensure your triggers are in the correct order—the essay variable equaling the text variable should be at the top of the list, which means it comes first.
Add another essay slide and repeat the process for every text entry question asked on the Likert survey slide.
Format your Likert survey slide as desired.
You may want to consider "concealing" your essay slides so they aren't seen by the user, even if the module goes a little slow when recording their answers. I covered mine up with a screen saying "Please wait. Recording Answers."
...And that should work! The result is a seamless survey that looks like one slide on the user's end! I hope this helps!

Awesome - glad that helped Steve and let us know if you need anything else.

hi i have an issue on the number of questions i can place on my likert quiz? i have 15 questions and i'm stuck at 10. Any ideas?
Hi Niño and welcome to E-Learning Heroes :)
You are correct, 10 is the limit per slide. I'd recommend splitting your questions across 2 slides if needed.
Perhaps others in the community will be able to chime in and share how they were able to overcome this limitation.
