I was able to see the same behavior in your file. Curious if you have it set up this way to force the slide to restart on purpose vs using the built-in attempts of the graded question slide?
I do not get the behavior when I adjust the attempts, so not sure if that will work for you in the meantime, but I for sure want to do some more testing to get this reported to our team.
PS - glad I had Zaxby's last night to avoid any cravings :)
It's been a while, but I think the final fix was to send the user to a blank screen and then have that screen instantly go back to the original. Reset Initial state should have taken care of it, but this at least got me going again

I was able to recreate the issue and identify where this has been reported to our team.
I'm going to add this thread to the report as we track user impact.
The workaround identified was the same and recommendation is to utilize attempts.
