I have created some question banks and for some reason some of the multiple choice answers are lying on top of each other when they display to the users.
I had this happen to me when I animated the answer choices. When I previewed the question, I saw that this had occurred. I went back to look at the answer choices in the timeline. I noticed that Storyline had added an addition animation to the Multiple Choice object. Sometimes this only occurred after I had saved the project and reopened at a later date. I solved the problem by going in the Multiple Choice object on the timeline and hid the extra animation for each of the answer choices that Storyline had created. When I previewed it after hiding the extra animations, it displayed correctly.

It may be helpful if we could take a look at your .story file if this does not alleviate your issue.
Hi Guys - thanks for the input, but I don't think that's the reason - there is no animation and I can't see any additional items in the timeline.
