The good news is, it looks like the video displays correctly when published. Check out my published output here.

I do see where this issue has been reported to our team regarding the Preview displaying incorrectly. I'll add this thread to the report as well as your sample file for our team. This will also allow us to update you here when we can.

I like the TV Jeff. 

Experiment using an alternative video type. Like .mov if you're using mp4, etc. 

Also check for SL3 updates.

One of the fixes addresses an issue where lots of videos in a course could cause slides to lag or lock up in HTML5 output, and they could be sized or positioned incorrectly during preview.
