I have made it so the user must exit the course if they have failed the test three times. However, still not passing the score to LMS after 3 attempts and course status is set as in-progress instead of "failed & complete"
What are you using as your LMS Reporting Option? It sounds like Passed/Failed may be the best choice for you.
If you're already using Passed/Failed, I would try testing the output in another LMS environment. Give it a try in SCORM Cloud, and let me know if you see any difference there.
