My course has a results slide with a failure layer and a success layer.
In update 9, if a user fails the quiz and subsequently passes, both layers show and play their audio tracks. The fix was to set the slide base layer to "Reset to Initial State".

If you've got the files you could also email them to Support(a)articulate.com and that'll generate a case as well. I haven't heard of issues with our submission form, but I'll check with our team.

Took some time to create the demo for you. When I submit it I get: "Whoops. We are having trouble processing your request. Please try again later."
