Numeric entry issue

Hi guys

I have a slide with 5 numeric entries. If the learner enters all 5 correctly, they should see the 'Correct' layer. If they enter any one incorrectly, they should see the 'Incorrect' layer.

I have done this successfully on one slide, but have repeated this on another slide and am not seeing the correct layer!

Both triggers are in place with Conditions set.

I have uploaded the slide with the issue, the orange boxes tell you what you should be typing in and the arrows point at the empty numeric entry boxes - please can anyone help me? It's driving me nuts!
