I have created a Pick One scale. When you preview the slide, you click on one option and the state changes to show it is selected. But if you change your mind and want to change your answer, when you click another one the state on the first selection still shows as selected. In fact you can click on all of them and they will show the selected state.
You'll want to include them all as a part of a button set. That'll help Storyline know that you can only select one choice. You can read more about that here.
