Printing Survey and Likert Scales

Hello! 

I am setting up a print-survey page, and I'd like to print the results of a likert-scale survey. When I print, however, the question is the "instructions" and the answers are all grouped in the "student answer" column.

Is there anyway to change this so each statement and not the instructions are set as the questions? 

I've attached the results page that prints and a copy of the set-up screen.
