Problem with Drag n Drop

I've continued to have problems with this drag and drop question. When I get the answer correct and then review the test at the end, the review shows that I got the question incorrect.
What would be causing this? You can view the question in the attached story file.
Regarding a different issue with this question, I tried to get the modules on the left to reset back to the left after the first incorrect attempt. I gave up trying to get them to reset and just had the student rearrage the modules. Does anyone have an idea how to get them to reset on the left for a second attempt at matching on the right?

One thing I notice is if you select the image on the far right, its outline overlaps the image in the middle. That might be clouding the issue.
I tried recreating the drag and drop inside of your story file on a new slide, using your images and it worked fine. Try recreating it on a new slide and see what happens
Do you have the slide set got reset to initial state on revisit.
Thank you for your suggestions. I do have the interaction set to go back to it's initial state after Submit.
When you match the modules correctly, you get the correct feedback. It's just when you go back and review that it shows the questions was answered incorrectly.Why would it register correct initially and then incorrect when reviewing? Sounds like I'm going to need to take a different approach to this question all together. But just curious what's causing the problem.
