Prompting for name on Results page

I selected the option to prompt the learner for his/her name in printing the quiz results, but this is what appears. Is there a way to format the text so the learner knows what is needed?

I figured it out - it's the Popups text colors. The text is white as well as the background.

Haha! Thanks for popping in with an update Lucy! I was thinking you may have deleted the text labels somehow and that wouldn't have been an easy task. :)
