Query regarding a published content

Hi Heros,

 

Need your help in one of the curious case happened with us.

 

Case

 

1. There is a SCORM package with an assessment in it.
2. All the users got correct marks out of 100
3. ONE USER GOT 125 out of 100 marks and this never happened again even if we checked on SCORM CLOUD, our own LMS and normal browse.

I am curious what could be the ROOT CAUSE in this case.
