Question bank freezing

I have a course that was originally built in SL3.  It was translated to Spanish and published with the most recent version of SL360.  The course has a question bank, 10 questions, randomly pick 5, results slide. 

Upon reaching the question bank slide, the entire course freezes.  I also had to remove the reset.results trigger to the slide before the question bank because that was causing the freeze as well.

Here's what I have tried...

1) Downloaded the trial version of 3 and tried republishing

2) Checked all triggers including var names

4) Taking things out to check for bugs

5)  Rebuilding the question bank

6) Had several team members try publishing

I know this was somewhat of an issue a few months ago when retrying a question bank slide.  FYI - although we no longer support Flash, it works fine if only published to Flash.

Any thoughts?
