Question bank not allowing input on 2nd attempt

Hi There,

I have created a question bank that allows learners to have 2 attempts. However, on the 2nd attempt it is not allowing me to select an answer. It worked before but now it's not working. I have already changed the settings on the question to 'reset to initial state' when revisiting. 

In preview this scene it works fine but when i select preview entire project it doesn't work.

Do you have any other ideas / suggestions,
