Question responses appear in the wrong place

Hi.  I'm hoping you can help me solve an issue.  I have a question master slide that looks like this:

When I create a new question, or import questions from a text file, they look like this, with the choices appearing in the upper left:

After selecting Apply Layout > Question, everything is formatted correctly.  If new questions use the Question master slide, then why isn't this formatted correctly from the start?  My story file is attached.  Thanks!
