Quiz Based on User Input

Hi there,

This is my first post - hoping someone might have some ideas on how to accomplish the following:

I need to build a quiz that displays questions based on use input. Here's what I'm dealing with:

The learner needs to indicate what chemicals they use at work. Each chemical is attached to its own question bank. Once the learner indicates what chemicals they use, a quiz is compiled of all of the applicable question banks (the ones that are linked to the chemicals they use).

I also need this to score some how.

Any ideas out there?
