Quiz Content Not Showing in a Large Size Project

We uploaded a course (16MB) in the SumTotal LMS. The course functions correctly during testing. After we launched it, most of the users are able to complete the course but we are getting random error reports from users that the one page quiz will show but without the question content. So the users are stuck in the course because the selectors were not displaying. After these users exited the course and retook the course, the problem went away...
Any ideas what might cause this? I couldn't replicate the issue to begin with. (FYI - we have about 1400 completions, and 5 error reports of this sort. The course just launched and we probably will get more error reports.)

Hi Yao!
You may want to test the content in the SCORM Cloud to see if you are able to replicate the error there. Also, it could be a browser specific issue, such as a security restriction. Do you happen to know where the affected users were accessing the content from?
Thanks Emily! I am not able to test outside my company system. The users should access the content from the same environment (same browser, same security setting, etc.)
I am guessing it's the internet connection, but I can't be sure.

If you continue to get the reports, you could always share the file here for us to test in the SCORM Cloud to try and reproduce. You could also enable the LMS debug log to see if any errors are happening from the back end.
