Quiz Not Advancing to Results Slide

I'm creating some training modules for a client. Each ends with a 10 question quiz. 

Module 1 went live in their LMS just fine. I copied the functionality from module 1 for the consequent modules. Everything is functioning correctly except for the quiz. 

The final slide will not advance to the results (hence, they are not able to capture the score/completion information).

Anyone have this problem before? Any idea why this would be happening all of a sudden? We have tried every variation and can't figure it out.

I've attached a screenshot of what it's doing in their LMS...and the .story file if someone wants to take a look. I have a feeling it's a simple glitch or setting, but has been incredibly frustrating.

We're over a month behind on our timeline for this project and I can't move forward without figuring this out.
