Quiz Not Continuing or Showing Up in Menu

I'm working with Storyline 2 on a project for a client and I've created a question bank for their quiz. It has been working fine until today when suddenly I can't seem to get the quiz to publish with the rest of my project. When I preview the project, the quiz doesn't show up in the Menu and it skips right over it. Any thoughts on what's causing it to behave this way?
One other quick question: before I completely lost the quiz during preview and publish, I published the course and the quiz would show up but only allow one question and then would move on to the next frame, past the rest of the quiz questions. Am I missing a setting that would create this? I have 8 questions total, each has a response to a correct or incorrect or try again submission.

I do have a question draw slide that has worked properly up until this point. I did reset the player properties and even deleted and re-added the question bank with the same issue. I un-clicked the "Draw questions randomly."

Hi again!
I figured out what my issue was. I had the "Include in Shuffle" on "Never" because I thought that meant it would shuffle it even if I had the questions locked. I changed it to "Always" and that was my issue.
