I have put together a quiz where the delegate has to achieve a pass rate of at least 80% to complete the quiz.
If the delegate doesn't achieve this then they have to 'restart the quiz/course', but when they restart the quiz the answers are already populated and unable to be changed. They are also unable to move off the screen - so technically they are 'stuck'.
make sure that the button "retry the quiz" has the trigger "Reset result x". I would guess that if this trigger is missing, that is what could result in what you are describing.
