Quiz questions repeat after being answered

I have a course with a quiz in it. After the user completes every question, it starts showing questions that have already been answered, with prev/next buttons instead of "submit". (But it does not show the correct/incorrect bar that I would expect in a quiz review.) If the user clicks through all the answered questions, it just shows them again-- it never shows the Results slide (which is supposed to come after the quiz).
I'm not sure why this is happening. I thought there might be a rogue Review Quiz trigger somewhere, but I can't find one. I also thought the slide draw might be calling for more questions than the question bank contained, but that's not it either. Does anyone know what is causing this?

Please take a look at the attached screenshots and story file. You need to add a new slide for Results. Please check Review Quiz option.

Thanks for your response. I tried adding a new Results slide, but it didn't solve the problem of questions being repeated after they've already been answered.

I took a look at Adrienne's case and it looks like her triggers were the culprit.
