On the attached quiz, the results page isn't calculating. I'm sure it has to do with the Success and Failure layers showing as unassigned but I don't know how to fix it.
Would it have to do with slides 1.6 through 1.9 having two answers but only one answer box? If so, how can I reformat this to make it work?
Finding this still to be an issue today, but your tip on duplicating the broken results slide to fix it worked great for me -- thanks! Wouldn't have ever thought of that, as I would've expected it to just duplicate the broken version.

looks like you have a corrupt results slide.
Your variables list looks like this
and it should look like this with 4 system generated result variables. That is why your success/fail layer triggers are 'unassigned'.
Move the existing Result slide to a new scene and then Insert a new Result slide and hopefully SL will generate the required variables.

Thank you, this fixed the issue.
I recently found this happens a lot, duplicating the broken results slide will fix it.
I had deleted the results slide a few times and re-added it but that didn't fix the problem.
When I moved the incorrect results slide to the new scene then re-added a new results slide it fixed the issue.
