Quiz Success not being recorded

Hello,
I have a course that is relying on a quiz to say if it is completed or not. The .story file is published as LMS - Tin Can API. Right now, the LRS is not getting the information that the course is complete when the quiz is passed. It does register that the quiz completes, and shows the score earned.
It does add a record that the course was "failed" when the quiz isn't passed as well. Just no record for the course success.
Does anyone know why this would happen?

Hello Tom,
While publishing the course which option have you checked for(in Reporting And Tracking) is it track using number of slides viewed or the result slide ?
Try publishing it with the Result slide it should work !!

Is this an issue that you are able to replicate in SCORM Cloud? This helps determine if the issues lies within the course or the LMS.

Thanks for the helpful responses. Sorry this is late, I wanted to gather as much information as I could before replying. I have the module Reporting to show completion on quiz success.
Currently, I can reproduce the behavior on SCORM cloud in the following steps:
The problem is between steps 4 and 5. Students are confused that they have passed the quiz, but it is not updating that status on their display, and it isn't intuitive to relaunch the course and close it again to see the completion. Also, we are basing their ability to move on to the next course on this data, and if they don't know to relaunch, they can't continue.
I'd be happy to share the .story file. Maybe I have configured something poorly.
