I would do the letters as drag and drop, and each letter gets dragged to a hotspot (maybe an underline or something indicating to the learner where each hotspot is?).
I have attached a little game I made for my great nephew. It has his great grandparents telling him he did a good job when he puts the letters in the right spot.

I don't have a template for this. Sorry! I'm picturing a square with a 'letter bank', then a series of hot spots in a row below. Insert a free form drag and drop question. Tell it which letters should go to which hot spot through the free form.
