I have just published my first e-learning course using Storyline.
I have a final quiz and an evaluation form, both of which require reporting, however I can only find a way to report on 1 results slide when publishing.
Could you please help with how to add a 2nd report to this please as we cannot currently see how learners have evaluated the course, only the results of their quiz.
In my experience most new Storyline users aren't aware of the issues in trying to get evaluation data into and out of an LMS. Survey questions are really easy to use in Storyline but that ease of use doesn't extend to most LMSs - unless of course you use Articulate Online :-)

Unfortunately, most LMS systems can only report one or the other. You should seek assistance from your LMS vendor.
Storyline does output both in the cmi.suspend_data but your LMS most likely will disregard the survey data.
A workaround could be to setup a Survey platform (like a Google form or a SharePoint Survey) and display the post course survey in a WebObject.

You don't need two results slides, you could add the survey questions to you assessment or use a results slide of results slides.
Some LMS's have issues with survey questions, particularly likert data
