Reporting on free text fields

Hi Team

I have a tricky reporting challenge that i am hoping to get some help with.

I have a module which requires two free text responses before selecting the submit button.

My question is - does anyone know how i can change the settings in my course (see attached) so that the fields entered into the module can become a reportable field in a elearning system (we are using scorm cloud).

 

My desired output would be a report that lists the uses name, course name, date course was completed and response for free text 1 and response for free text 2.

Does anyone have this/know how i can create it. I realise i will need to work with our system developers to get the report. - But first i need the course to be capable of exporting the answers.

Can anyone help?
