I am new to SL3. In the Drop-Down list question type (Matching and Sequence), how can I setup a button which can allow users to clear answrs they have entered (before they click on Submit button)? The slide property has been set as "back to the initial state when users revisit". However, when users click on the rewind button, the answer still remains there.
"Reset to initial state" option will only reset your slide and not the variable. You need to create a trigger to "when timeline starts" that tells the variable to equal a value of zero.
Also, you need to add similar triggers to reset your other variables to their initial values as well.

Anyways, after checking your file I have managed to create a work around for a reset answer button in the drop-down list question type (sample button is on 1.10 Post-Test slide) to work. Here's the steps that I've applied:
1. I've added a blank slide to the scene (1.9 Untitled Slide) and a button (reset answer) to 1.10 Post-Test slide.
2. To the new blank slide, I've added a trigger to "Jump to slide" at "start of timeline" - it's jumping back to the Post-Test slide.
3. On the 1.10 Post-Test slide I set the "reset answer" button to jump to the blank slide.
Not really sure if this is the exact thing you want, but you may want to start on the solution I've made. You can apply the above step on the slides that you would want to have the reset answer button.
I have attached my revised file here.

If you want to apply it on your other slides - my answer is yes, you need to have 1 blank slide each. Because each blank slide need to have different triggers - jump to a specific slide, so you can't use one blank slide only.
