I am having 2 related issues with a simple quiz. My course allows for 3 attempts before failing. With an approval email from the person's manager, I want to allow only one more attempt when he re-tries the course. How do I accomplish this?
My quiz also does not save the failed attempts if the user exits the course after 2 failed attempts, Instead when he returns and restarts the course, it is resetting attempts to allow for 3 more.

To allow one retry after manager approval, I suggest looking into your learning management system (LMS) settings. Most LMSs have a "lockout" feature that will prevent the learner from accessing the course after a certain number of attempts.
What LMS are you using? Hopefully, other folks using the same LMS can chime in with their tips!
To help troubleshoot why those failed attempts aren't being saved, I need a little more info on your quiz and LMS reporting/tracking settings. Are you able to share your file here so I could take a look? If you prefer, you can share it privately, and I'll delete it once done!
