"Reset to initial state" apparently not working

I have a response slide on which learners need to drag and drop numbers to match categories. Learners have three opportunities to pass the quiz.

The first time learners access the slide in question, the responses are randomized. The second and third times, the responses are accurately lined up with the categories.

This happens when the slide properties set up with either "reset to initial state" or "Automatically decide". I checked this by completing the quiz using the "Preview" feature and by publishing it.

What might be causing this slide to not randomize the second and third times through?

I've attached a screenshot. The items on the right are the pieces that should be appearing in a random order.

I tried working locally – same issue.

The reset button is on the results slide. The “jump to” slide is at the end.

I’ve attached a file with the slide having issues (slide 2) along with the results slide.

Hello Diane!

Thank you for sharing the file. I was able to recreate the issue and see that this has been reported to our QA team on other types of questions. I have added this information, and will share any updates as we get them.

One workaround would be to close out the course before retaking the exam, and resuming where you left off. This does shuffle the answers for 2/3 attempt.
