When I choose all of the correct answers, the Results Slide says that I've score 17.24% (25 points). The Passing Score is 80% (116 points). I can change my answer options, and I get the same score (17.24%) on the results slide every time.
This is happening in multiple courses. These courses were originated in SL1 and upgraded to SL3. Not sure if that information is relevant.
if you don't mind it being here in the public forum, just use the grey 'Add Attachment' button at the bottom of the post to browse and upload from your local drive. it's the .story file not the published file we'd need.
If you can't share publically, you'd need to submit a case with Articulate and share it privately.

Hi Karen! Those are excellent first steps, so kudos for going through that troubleshooting process.
Tell me more about the quiz not working. Is it not aggregating a score on the results slide?
Is it not reporting to your LMS?

The score (17) is reporting to my LMS.

Can you share the .story file - might be easier for us to help you troubleshoot if we can see the file setup.
