Just did some further digging. If I delete all existing questions and added a test one whilst leaving my "custom triggers" as is. The pass and fail behaved as required.
From that I'm taking it that my questions are somehow "corrupt".
Any thoughts here? Am I faced with re-typing numerous MC questions :(?
