We worked out the issue. We are using a custom template with a custom review. The template defaults to certain radio dials, so if the correct answer didn't match the template answer, it would show the incorrect layer. Once I adjusted the trigger, that resolved the issue.
And yes, I had checked the reset to initial state and the timeline triggers; both were correct.

How are the slide properties set on your question slides? Are they set to 'Reset to Initial State'? I've seen this cause difficulty during review.
