SCORM output help

I need a little help with the SCORM outputs from Storyline and how I can change things like the Question No and Question title.

I have attached a PDF output from my LMS which shows the detailed answers from a Survey I have been testing. You will see on page 3 the Question No and Question fields. It shows the details from Stroyline however they are of no use to me in that format. Is there any way to change what it outputs as I cant find anything?

You will also see on page 4 the answers from the questions. You will see that the Likert Scale question has grouped all of the answers together. Is this a SCORM/Storyline issue or an issue with my LMS? If it's Storyline/SCORM is there any way to change it?
