SL2Question Bank Skipping Q's Randomly

This is a 13 question quiz. Questions are skipping randomly for different users. Some users are reporting receiving 2 questions, others 5 and some have seen all 13. I have tested the course in tempshare and sometimes have experienced all the questions twice and 2 questions 1 time, and 7 questions 1 time. At my wits end!

Hi Ron!

I can share it . . . I built all the questions. . . None were imported. 

Another issue is that it is not calculating the final quiz score correctly. It has 13 questions worth 10 points each and set to pass the user at 80% (104). It was previously set for 85%. If a person misses 2 questions that should be 110% which is passing. The attached user quiz report shows questions which were answered correctly and show correct in the attached printout yet scored incorrectly when reviewing the quiz after completion. Question 1, 2, and 3 were answered correctly but showed incorrect during review and are causing a fail score for the learner who should have a passing score because she has 110 total.

I noticed that your last slide in your bank has been edited to jump to the result slide.  This should say "Next Slide" like all of the others.  Since you have these in a bank and all of the questions are set to shuffle, then your quiz automatically ends when this question appears, since this trigger will take the student out of the bank.  If it hapoens to be the first question, then you only get to answer 1 and you are done.

Correct this and I think you may be golden.
