Storyline 2 - Yes/No Questions

I am looking to put about 5 Yes/No questions onto one slide, as I have 15 yes/no questions and each answer should be graded and go towards the final certificate at the end of the training module.

What I am looking to do is have a question then 2 check boxes next to the question (one for 'yes' and one for 'no'). Does anyone have any ideas on how to do this?
