Thanks for reaching out, and I"m sorry you're unable to add a quiz question to Storyline 3! Does Storyline freeze when you create a new file and try to add a quiz question? A few tips to help troubleshoot:
Check that the files are saved locally. If not, save them to your C drive and then give it another try.
Make a copy and rename the file.
A repair of Storyline may also get you moving in the right direction if all else fails. Let me know how it goes!
