Storyline 3 - Last Question - won't allow person to submit incorrect answer

I have 4 questions in a Storyline module.     The last questions is true/false.

When I publish it, nothing occurs when the person selects the incorrect answer.

I took the question out, put in another slide with a true/false and it had worked in the preview.  When I published, it no longer responds.

There is a true/false question earlier that is working correctly.

I am using the default submit button on the player

Does anyone know why this might be occurring?

Thanks so much for looking at it.  I am sure it is something obvious, but not seeing it.

Attached is an early version.  In this one, nothing is happening even in preview when you choose either true or false.     

Hopefully, we can fix this or I can send the one I thought I fix when at work tommorow.

Thank you so much.   Was also able to resolve the original problem also.    I had left the old version of the question in, right before the one that was corrected.  So, while it looked like nothing happened - it was just going to the next question which was exactly the same.
