Also, is it a slide, or have you modified the report.html file (the Print Results feature)?
If it's a slide, you'll want to be sure you have a text box that includes a reference to the text entry field variable on the name entry slide. Also, make sure it isn't hidden, either with a Hidden initial state, or hidden on the timeline:

Slide 10.11 is the data entry slide.
Slide 10.13 is the certificate with the 'passing' layer.

just change the variable reference on slide 10.13 to be the same as the variable name from 10.11
