Can someone please take a look at me file here and tell me what I may have set up incorrectly? I have taken the course several times after loading to our LMS and the question are not shuffling. Will they only shuffle if I select the Retake Test button? Or should it shuffle each time the user takes the test?

It's hard to tell what's going on with the file that you've shared because the Question Bank information did not populate.
I have a couple of questions to better understand based on what I can see in your file:
Are the individual slides not pulling random questions or is the issue happening when you retry from the results slide?
Are you using the 'Start Course Over' button? I don't see a trigger to reset the results using this option as there is on the 'Retake Test Only' button.
