Students not able to change answers on quiz questions

Hello,

I've recently completed my first Articulate Storyline project, a series of mini-quizzes that are hosted in Blackboard. An issue has been found, in which the students are unable to switch answer choices on some questions prior to clicking submit. Instead, two choices become selected and the SCORM object marks the response wrong once submitted.

Can anyone provide insight on this problem?

here are the release notes for SL360 so you can decide if you are going to update to latest release

First thing I notice with this Multiple choice (one selection allowed) slide is that it is allowing more than one item to be selected 

When I checked this slide there is no button set selected - which is where the issue lies

This is question 2 which has the correct option selected - see the tick next to multiple choice.

So what you need to do is highlight the four responses in the first question > right click > tick Multiple Choice.  This means only one button can be selected at any time which is the meaning of a multiple choice question.

Thank for the detailed response, I really appreciate it. I have been making these changes throughout the document.

There is another slide with an issue- a  Multiple Response slide, the first slide in this module.

Although I have three responses checked as correct, the "Correct" answer doesn't come up when they are selected. I've looked at the button set, but Multiple Response doesn't come up as an option. Additionally, when the slide publishes, the box shading around some of the questions goes away. 

I have attached the file.

The button set doesn't apply to a Multiple Response question because you want the user to be able to select more than one option...so that is correct.

I see you have a custom submit button which is unassigned but the player (default) submit button is also active...which one did you want to use to submit the interaction?

I've encountered a similar problem in a Multiple Response slide, but can't change the button set b/c I'm not prompted for Multiple Response. See below if that is not clear. It is the first Quiz slide in M6.
