Submitting multiple text entry fields from one screen

Hello all. I have read every thread I can find on this and none are exactly what I need. I have a client that wants us to collect DOB, Height, Weight, Eye Color and Hair color from the student while taking the course. They want this data to push to their LMS. I have used short answer survey questions and they work, but they are clunky and I cannot have multiple text entry fields on one screen in order to create a standard data format, ie. DOB as mm/dd/yyyy, or height using one field for "feet" and one field for "inches."  Basically I need the student to enter data in multiple fields on the screen and have the answers push to the LMS...make sense?  Any help would be greatly appreciated.
