Survey question mixing "Pick many" and multiple "Short answers"

Hi all,

I'd like to create this kind of question in a survey quiz :

Select the trainings you have had previously :

[_] Formal In-house training provided by employer

[_] Training provided externally (please name provider) ........................

[_] E-learning course

[_] Other (please state) .............................

The user will have to click on the answers that correspond to his own experience and complete the blanks if necessary.

Can you please tell me if it's possible ? And if yes, how can I do it ?

I have Articulate pro 09 with quizmaker09. 

I need  make the same question as Nadege. But the hyperlin isn't work :-(. Can you help me?

Hi Ashley,

I would also like to use the survey quiz tool for multiple response. I can't see how you indicate which answers are right and which are wrong. 

The survey quiz for one question seems very straight forward but the multiple response quiz does not.

I'd really appreciate any help.

Hi Sally! This thread is a bit dated. Can you share a bit more about what you are looking for? You mention survey, but want right and wrong choices - which indicates a graded question type. Also, what software are you utilizing? This is a Storyline thread, but just wanted to verify since there was a Quizmaker question above as well.
