Survey Results Not Reporting

I am running into issues with building courses with survey questions that need to be reported. The module I'm working on currently is a series of informational slides followed by two pick-one questions and then a Survey results slide. When I test the module on SCORM Cloud, the log does not show any responses. Are there any known issues with building surveys that don't report? I've searched through the forums and am not finding anything that specifically resembles the issues I'm having here.

I just published your course, uploaded to SCORM Cloud, and did not get the same results:

Oh wow...that's awesome, but I'm at a loss of why it is doing this on our side. Can you share the settings you used when you published so I can ensure I've got the same thing? Or did you use what I already had?
I didn't change a thing John, just did a publish to LMS and uploaded the zip file to SCORM Cloud.
Thanks for the info, Leslie. I did the exact thing, and the debug log is what I came up with. I'm at a loss, but I really appreciate your help in testing the file.
Have you tried doing the upload/test again? Really strange that we would get varying results for sure.
We've been getting similar results in the LMS as well. I've tried re-building courses and replacing slides, but it is a random issue that pops up. Unfortunately it popped up on a course meant for a LOT of people and I need to ensure it doesn't occur again.
