Survey slide will not display

I'm trying to create a sequence of content and question slides:
- Intro slide on the topic
- Series of quiz question slides
- A few content slides on an agreement users need to make
- Survey question slide where users click to agree to comply
- Results slide
No matter what I've done and what order I've placed the slides I cannot get the Survey slide to display. If I put the items in the order above I can't open the survey slide after the final content slide. The content just freezes at that point. I've tried using a quiz question instead and it still freezes. I've tried placing the quiz and then the single survey question in separate question banks. Still no luck. When I tried putting all of the content at the beginning and adding the survey question to the top of the question bank it just skipped over the survey question and went to the first quiz question. At this point I'm stumped. Feels like the Twilight Zone. Any thoughts would be appreciated.

see sample .story file attached done in SL1. Should the survey be part of the results or just the quiz questions. In my example its not included — I am able to see all slides in the order that you have above — or maybe I am not understanding your issue.

The survey slide does need to be included in the results because it confirms that the individual agrees to the requirements and that information is tracked in the LMS since the content is scormed.

I am having issues with SL2 so I have updated my SL1 sample and it works as expected with the survey included in the result slide.
Hoping to have SL2 reinstalled later today and then I can check your file but in the meantime can you check the survey options: Do you have the option Require: set to User must answer

I checked your file and i could not get the slide in question to open in preview mode by itself or through an entire preview.
I duplicated a quiz slide and added the question/answer in for the agreement. Everything seems to be working as expected now.
